Hello and thank you for your interest in becoming a member of PEER. To start the process, please complete and return the PEER Membership Application which is available for download as a Word document, below. Complete and fax back to the fax number noted on the application.

Upon receipt to the PEER Membership Group, your application will be reviewed and presented for acceptance.

As part of the the application process, current PEER members will be requested to submit experiences they have had with the applicant, whether positive or negative. This information will be shared with the applicant for any necessary clarification, particularly anything that could derail acceptance.

After review, if the applicant passes this stage, the application will be submitted to majority vote for acceptance.
